How to make a referral
Self referral
Complete an Inquiry, call 0426 851 396 or email admin@sunflowermentalhealth.com.au.
Doctors and other professionals
Email the referral letter and Mental Health Treatment Plan, if applicable, to admin@sunflowermentalhealth.com.au, or fax it to 08 8271 4421.
Please include the reason for referral, relevant funding or rebate information, and the patient's contact details. We will contact your patient within 48 hours, then send you a referral acknowledgement after confirming with them.

Medicare
Australians with a valid Mental Health Treatment Plan and referral may be eligible for Medicare-funded appointments, with access to rebates for up to 10 individual sessions each calendar year.

Sunflower is a private, non-government-funded service. You pay the session fee and Medicare covers only part of the cost. Limited reduced-fee places may be available to clients referred through specific pathways who hold both a valid Health Care Card and Mental Health Treatment Plan. Payment details are required under the cancellation policy. Please contact the practice to discuss eligibility.
Private health
Benefits vary by insurer and level of cover. Please check with your private health fund.
NDIS
Self-managed and plan-managed NDIS participants can enquire where psychotherapy aligns with plan goals. NDIA-managed participants cannot currently be accepted.
DVA
DVA funding depends on eligibility, referral requirements, and service fit.
WorkCover
WorkCover funding depends on approval details, treatment goals, and service fit.
How is the gap fee calculated?
The gap fee is the session fee minus the rebate or benefit you receive. Medicare rebates can change, and your actual gap depends on eligibility. Benefits from private health, NDIS, WorkCover, Teachers Health Fund, and DVA also vary.
